Notice of Material Modifications to the Day Care Council - Local 205,

D.C. 1707 Welfare and Group Legal Funds

Dear Welfare Fund Participant:

 

Due to financial conditions that are beyond the control of the Day Care Council - Local 205, D.C. 1707 Welfare and Group Legal Funds (“the Funds”) the Board of Trustees have voted unanimously to approve the following changes to your benefits effective January 1, 2006.

 

Optical Benefit (Active and Retirees) - Available once every 24 months from last date of service.  Previously the benefit was provided once every 12 months.

Survivor Benefit (Actives and Retirees) - No longer provided; a life insurance benefit may be available through the Cultural Institutions Retirement Systems (CIRS).  Their telephone number is (212)674-0101.

Tuition Assistance (Actives) - No longer provided; members may continue to apply for EIP grants through the State University of New York Educational Incentive Program, Early Childhood Program.  The telephone number is (800)295-9616.

Supplemental Disability Income (Actives) - The supplemental Income available months four through six for approved Disability Leave is no longer provided.

Blue Cross Extended Rider (Actives) - The Extended Benefits Rider provided by the Welfare Fund to those participants enrolled in GHI/Blue Cross is no longer provided.  Accordingly any hospital stay will be under the term of the basic 21-day Blue Cross contact.

Health Care Allowance (Retirees) - Partial reimbursement for Health Care Insurance premiums is no longer provided.

The Prescription Plan is modified as follows:

$100 Individuel/$200 Family Deductible - Active member (or dependent) will have to meet a $100 deductible before prescription coverage is available then a second or combination of dependents will have to meet an additional $100 deductible (maximum of $200 deductible per Family per calendar year) before prescriptions are available to a second person and/or family.  The Medco Identification Cards must be shown at participation retail pharmacies, in order to track and credit deductible.

Mail Order - Co-payments are increased to $20 for generic drugs, $30 single– source brand (drugs with no generic equivalent) or $50 multi-source brand (brand drugs prescribed that have a generic equivalent) for a 90-day supply.

Retail Pharmacies - Co-payments are increased to $10 for generic drugs, $20 single-source brand (drugs with no generic equivalent) or $35 multi-source brand (brand drugs prescribed that have a generic equivalent) for a 21-day supply.

The Annual Maximum is changed to $5,000 per Calendar year per person.

 

The Board of Trustees was compelled to make these changes in order to maintain the long term financial strength of the Funds so that they will be able to continue to provide benefits for you and your dependents.

 

Please attach a copy of this notification to your Summary Plan Description (SPD) for future reference.
